If you are an existing patient you will be treated for your No Fault case. Please see downloadable forms below to complete and bring to the office completed if you are involved in an MVA.
Before your first visit we ask that you have your medical records sent from your previous office. On your first visit, we will ask you to arrive early with a current list of your medications and your preferred pharmacy as well as a list of any other MD’s you are seeing like a Cardiologist, ENT etc. You will be asked to turn in the forms below. If you have not completed them you will be asked to do so at that time.
Be sure to bring your insurance cards to each visit. We will take your picture to prevent insurance fraud at your Initial Visit.
