Orchard Park Family Practice P.C. has been awarded recognition at the highest level by the National Committee for Quality Assurance (NCQA) for Physician Practice Connections-Patient Centered Medical Home.

The Patient Centered Medical Home (PCMH) is a health care setting that facilitates partnerships between individual patients, and their personal physicians, and when appropriate, the patient’s family and other specialties. Care is facilitated by registries, information technology, health information exchange and other means to assure that patients get the indicated care when and where they need and want it in a culturally and linguistically appropriate manner.

The concept of PCMH gives Orchard Park Family Practice the opportunity as a practice to not just treat our patients on an as-needed basis for illness or injury, but also allows us to create a care-coordinated, long term healing and preventive relationship with our patients. It gives our patients a sense of trust and security in our practice as a whole knowing we have and will continue to put forth the extra effort into their continued care.

We are very excited to announce Jill Kaczmarek, FNP to our Team! She is joining our practice as a Family Nurse Practitioner on November 4^th and will be available for scheduling in December.

Jill graduated Summa Cum Laude from D’Youville University with a Bachelor of Science in Nursing and subsequently Master of Science in Family Nurse Practitioner in 2000. She joins the practice with 25 years of experience in Pediatric & Adolescent Medicine. Jill was inducted into the Sigma Theta Tau Honor Society and is a past President of the WNY Chapter of NAPNAP Association. 

Jill is a lifelong resident of Western New York. Outside the office she is avidly into fitness both in gym and outside on the bike, traveling, fashion and the world of cosmetic injections.

We are very excited to announce another new Provider, Mandy Abram, PA to our Team!

Mandy is joining our practice as a Physician Assistant on October 14th and will be available for scheduling in November. 

Mandy graduated Summa Cum Laude from Daemen University with a Bachelor of Science in Health Science/Master of Science and Physician Assistant Studies in 2011. Since then, she has worked in primary care. She enjoys developing relationships with her patients and has a passion for health and wellness. She’s a mom of 4 and loves staying active.
