WHAT TO EXPECT AND WHAT TO BRING

To request acceptance into Orchard Park Family Practice please call 662-5357. At that time you will be asked some basic information including your insurance carrier, if any. This is so our billing department can verify participation.

Orchard Park Family Practice does not accept any Worker’s Compensation cases for new or existing patients, nor do we accept New Patient No Fault cases. If you are an existing patient you will be treated for your No Fault case. Please see downloadable forms below to complete and bring to the office completed if you are involved in an MVA.

Before your first visit we ask that you have your medical records sent from your previous office. On your first visit, we will ask you to arrive early with a current list of your medications and your preferred pharmacy as well as a list of any other MD’s you are seeing like a Cardiologist, ENT etc. You will be asked to turn in the forms below. If you have not completed them you will be asked to do so at that time.

Be sure to bring your insurance cards to each visit. We will take your picture to prevent insurance fraud at your Initial Visit.

Making an Appointment

• Orchard Park Family Practice is unable to accommodate walk-in visits. If you have a life threatening illness or injury please call 911. We do have same day appointments available. Please call as early in the morning as possible to make ensure availability.
• We require advanced notice if you must reschedule or cancel an appointment. Patients who miss two appointments without the proper notification may be dismissed from the practice.
• Our office will call you 48-72 hours in advance to confirm scheduled appointments. Please arrive promptly for your scheduled appointment. It will be necessary for you to reschedule if you are more than 15 minutes late.
• Our physicians’ office hours vary and not all of them are available for evening or Saturday appointments. Appointments for annual physicals are available at pre-determined times according to the provider you wish to see.
